How are applications submitted?
Applications must be submitted online through the Center for Scholarship Administration (CSA) website. There are three steps to complete the application:
1. Click the "create application" button below to answer the prequalifying questions. When you submit these, it will automatically take you to the profile page.
2. Create or update your profile. Save and continue to the online application.
3. Complete the online application. A confirmation email will be sent when the online portion of your application is complete. Applicant should contact CSA if a confirmation email is not received.
4. Mail in, or email, the packet of additional required materials. See "What constitutes a complete application" below.

When is the deadline for submission of materials?
The online application must be completed and transmitted to CSA no later than . Any required supplemental materials that need to be mailed must be mailed to CSA no later than and should be mailed in one package.

What constitutes a complete application packet?
A complete application package includes the following:
• Student Authorization and Acknowledgements Form, found in the online application, signed by the scholarship candidate. Download the Student Authorization and Acknowledgements Form.
• Official transcript with grades and cumulative GPA posted through the latest semester.
Submit a TWO LETTERS OF RECOMMENDATION; one from your caseworker and one from a person of your choosing.

Submit an ESSAY 2 Submit a maximum two-page explaining where you see yourself one year from now and five years from now. What are your plans for this one and five year timeframe? How will the Huth Scholarship help you achieve your goals during these timeframes?
• List of academic honors, leadership skills, community service, etc.
Submit a copy of the Millennium Scholarship.
• Student Aid Report (SAR) from the Free Application for Federal Student Aid (FAFSA).
• W8 or W9 IRS Form, as applicable. This form must be completed by the applicant.
- What is the value of the scholarship award?
Annual scholarship amounts will not exceed $10,000.
Huth Scholarships are given for post high school education for a maximum of five years, with the intent of the student obtaining an undergraduate degree or a trade school certification. No funds are to be used for post graduate education. Once a scholarship amount is awarded, it will not be increased from the original amount approved. - What schools can a recipient attend?
Students must be enrolled full time in a school located in the State of Nevada, and stay enrolled full time (except for summer school), as defined by the institution they are attending. Courses for which students receive an incomplete grade or a pass/fail grade do not count as completed courses for determination of full time status.
Unless a student experiences an emergency as described below, the student must continue his or her education without stopping. If a student otherwise terminates his or her enrollment, the Scholarship will also terminate and will not be reinstated. If a student must postpone participation in school due to an emergency, the following requirements must be met. - What if I am unable to attend consecutive semesters?
Emergencies:
In the event of a death in the family (defined as a foster parent, parent, grandparent, sibling, child or spouse) the student must notify Hanna Hamlin within two weeks of the death and forward a copy of the death certificate, when available.
In the event of a medical emergency, the student must notify Hanna Hamlin within two weeks of the onset of the emergency and forward a statement from the attending physician as to the nature and duration of the medical emergency, if reasonably known.
With either a death or a medical emergency, the student must begin full-time class participation at the beginning of the next period of instruction (excluding summer school) following the emergency. -
How are scholarship recipients selected?
A selection committee will consider the respective ability, educational goals, financial need, and career ambitions of each applicant.

How will I be notified if I am chosen as a recipient?
All applicants will be notified in writing by email, whether or not they have been selected to receive a scholarship. Those students selected will download a package from their online account that includes a scholarship agreement and information about the scholarship terms.

Is this scholarship renewable?
This scholarship is a five-year scholarship subject to the following terms and conditions: Students must remain enrolled full-time, continuously throughout the five-year scholarship. Under special circumstances, individual exceptions may be made to the full-time continuous enrollment requirement, at the discretion of the committee and Trustee. If a scholarship is awarded for an academic year, and is not used within that year, it is forfeited, and unused money is returned to the Trust. Failure to meet the eligibility requirements will result in forfeiture of the scholarship.
Scholarship recipients will receive information from CSA each year on the procedure to verify continued eligibility for the scholarship. Recipients must submit a renewal application, transcript or grade report, and W-8 or W-9 form, in order to be considered for renewal of their scholarships each year. Scholarship amounts are not guaranteed, as renewed scholarship amounts may vary from year to year due to a variety of factors.
